GuidanceCabotegravir is reimbursed in France and funded in Spain; NICE recommends it in England for people who can’t take oral PrEP. Lenacapavir has been EU-authorised since August 2025; national availability varies. Sources: WHO; NICE.

Dosing oral PrEP

Regimen	Schedule	Suitable for
Start	Two tablets 2–24 hours before sex.	Everyone, for every type of sex.
Daily	One tablet every day.	Everyone. Use daily dosing with chronic hepatitis B.
2:1:1	Two tablets before sex, then one a day for 2 days after the last sex.	Anal sex and any insertive sex.
2:7	Two tablets before sex, then one a day for 7 days after the last sex.	Receptive vaginal or frontal sex, and injecting drug use.

UK 2025 guidance. National rules differ: WHO recommends event-based dosing only for men who have sex with men. BASHH/BHIVA 2025

Tests and monitoring

Before starting

HIV Ag/Ab, hepatitis B and C, STI screen, eGFR, pregnancy test if appropriate.

Day one

Start while results are pending, unless there are symptoms of recent HIV infection.

Every 3–6 months

HIV test. STI tests every 3 months if new partners.

Yearly

Kidney function; more often from age 40.
